DAVIS VISION

The Lieutenants Benevolent Association, in conjunction with the Superior Officers Council (SOC), has introduced a new optical procedure through Davis Vision.

On September 1, 2007, all active lieutenants were mailed the new program information and identification cards.

If you have not received this information, call the SOC at 212-964-7500, to verify your address and to provide other information, if required.

Active members will no longer be required to obtain a voucher from the SOC prior to receiving benefits. All other vision care plan benefits will remain the same. If you have any questions, contact Davis Vision at 1-800-999-5431 or www.davisvision.com
